Procedure for turning in Disability Test Request Forms

Please remember to get your DSS Test Request Form in as early as possible. Space is limited & scheduling testing rooms is on a 1st come, 1st serve basis.
Please be sure to show your instructor a copy of your Accommodation Agreement during the first week of class, you will not be allowed to test until this is done. Students are responsible for ensuring that this form is completely filled out and returned to the Disability Testing office at least 3 weekdays prior to scheduled testing. Forms for Final Exams must be turned by a specific date each semester, please see date listed at the bottom of this page, or on the DSS Testing Final Exam Schedule found at the top of the page. You are allowed to turn in 1 late form per semester, this is considered an “OOPS”. After that if another late form is turned in, you will need to take the test in the classroom without accommodations. You cannot use your “OOPS” for final exams.
You are expected to take your test at the SAME TIME you would in the classroom, taking into account any extended time, unless you have a scheduling conflict or the instructor’s permission. GRCC’s Disability Support Services Office maintains a high standard of academic and personal honesty when providing testing accommodations. If a student is caught cheating—the test, notes, and any unauthorized materials will be given to the instructor and the Student Code of Conduct office will be notified.
Students: Please remember that it is YOUR responsibility to remind your instructor of your test date and time. This will ensure that your test is delivered on time. Students please be advised you will be monitored &/or recorded while in testing rooms.
Rescheduling a Test
If you need to reschedule a test for a disability related reason, please email your instructor and copy email@example.com asking for permission to reschedule your test. After we receive instructor permission to reschedule, it is your responsibility to contact the DSS Testing Center to reschedule.
MA-95/96/97 Retake Process
If you need to retake a MA-95/96/97 test , please follow the retake process below.
We will be allowing students to do drop-in testing whenever possible. You will need to bring a completed MA-95/96/97 DSS Retake Permission Form that has been signed by your instructor when you come to test & we will do our best to accommodate your requested testing time. There are forms available for pickup in DSS Testing, or your instructor will give you a copy.
