Lakeshore Disability Accommodations Request

Please show your instructor a copy of your Accommodation Agreement during the first week of class. You will not be allowed to test until this is done. Students are responsible for submitting the webform at least three weekdays before testing.

Webforms for final exams must be received seven weekdays prior to testing. Please note that holidays, weekends and school closures do not count as weekdays.
 

If you turn your form in: the earliest you can test is:
Monday by 12 p.m. Thursday
Tuesday by 12 p.m. Friday
Wednesday by 12 p.m. Monday
Thursday by 12 p.m. Tuesday and Wednesday


You are allowed to turn in one late form per semester — this is considered an "oops." If a second form is submitted late, we will not be able to provide accommodations. You cannot use your "oops" for final exams.

You are expected to take your test at the same time you would in the classroom unless you have a scheduling conflict or the instructor’s permission. We maintain a high standard of academic and personal honesty when providing testing accommodations. If a student is caught cheating, the test, notes and any unauthorized materials will be given to the instructor and the Student Code of Conduct office will be notified.

Please remember that it is your responsibility to remind your instructor of your test date and time to ensure your test is delivered on time. Please be advised you will be monitored/recorded while in testing rooms.

Mondays: 3-6 p.m.
Tuesdays: 9 a.m.-2 p.m.
Wednesdays: 2-6 p.m.
Thursdays: 8:30-10:30 a.m.
