Use this form for requesting the recording of an event or creating a production on behalf of your department. Please know that we will attempt to support as many reasonable internal requests as possible; if we are already booked for another event, we may need to hire freelance support to cover yours. If your event requires other technical support (a sound system, microphones, video conferencing set-up) please use the Event Support Form .  

You may appeal your Academic Suspension for the following reasons, however the acceptance of your appeal is not automatically guaranteed: Extenuating Circumstances Serious Illness Unexpected Economic Hardship Death in family Grade Dispute Students disputing a grade need to follow the Academic Grade Grievance Procedure . You must submit third party documentation to support your personal statement in your appeal. Examples of third party documentation are anything not authored by the student that further supports the extenuating circumstances that lead to suspension such as obituaries, medical documentation, letters of recommendation, etc.  Please note that students returning from Academic Suspension will be required to meet with an academic advisor prior to being able to register for classes.
