Before you arrive, make sure you have completed the DS-11 Passport Application (download at travel.state.gov).
IMPORTANT: DO NOT sign the application before arriving. You must sign it in front of the passport acceptance agent.
Proof of U.S. Citizenship
Bring ONE original document:
- Certified U.S. Birth Certificate (with raised seal)
- Naturalization Certificate
Original documents are required. Copies will not be accepted.
Government-Issued Photo ID
Bring ONE valid photo ID:
- Driver’s License
- State ID
- Other government-issued photo identification
Original ID is required.
