- A description of the concern or what was reported to you
- When and where the conduct occurred, if known
- Any relevant documents, messages, emails, photographs, or other information

- Options for resolution: Submitting a report does not require participation in an investigation. Individuals may request support, an informal resolution or a formal investigation at any time.
- Protection from retaliation: Retaliation for reporting or participating in an investigation is strictly prohibited and will be addressed promptly.
